Patient Information Section

Race

This is the patient's racial background. Select the patient's race by clicking all Race check boxes that are a race for the patient. Upon saving the patient with race(s) selected users will not be able to remove the race that was selected.  Please note that Hispanic is not considered a race. It is an ethnicity.

Ethnicity

This is the patient's ethnic background. Select the patient's ethnicity using the Ethnicity pick list.

School

This is the patient's current school of attendance. This list is derived from entering all associated schools into the system using the manage schools screen. Select the school using the School pick list.

Occupation

This is the patient's occupation. Select the patient's occupation using the Occupation pick list.

Mother's HBsAG

This is the patient's mother's HBsAG Status.  Select the patient's mother's HBsAG status using the Mother's HBsAg pick list.

Language Spoken

This is the patient's spoken language and will determine the language the reminder letter for the patient will be printed. Select the patient's spoken language from the Language Spoken pick list.
