Modify Vaccine Request

  VFC Pin

This information displays at the top of the screen,  this is the VFC PIN  for the  organization logged in.

 Initiating Organization

Displays the organization that started the order.

    Organization Phone

Displays the  Phone number of the  provider organization that started the order.

Organization Fax

Displays the fax number of the organization that started the order.

 Delivery Contact

Display the name of the delivery contact for the provider organization.

   Delivery Address

Displays the delivery address specified for the provider organization starting the order.

Delivery Days/Hours

Displays the delivery days and hours for the provider organization starting the order.                                                                                                                        

 Trade Name

This column displays the Trade name of the vaccine.

   Packaging

This column displays the Packaging Description displays all available units per package.

    Manufacturer

This column displays the manufacturers information of the vaccine.

Type

Displays the  type of vaccine e.g. VFC, Flu Pool and Pandemic.

Initiating User 

Display the name of the delivery contact for the provider organization who submitted the order.

Request Date

Displays the date the original vaccine request was submitted.

Minimum Order Quantity

This column will be the smallest order that can be placed based on NDC screen calculations (units per package x doses per unit).  

Original Amount Requested

The column displays the  original amount of the  trade name/packaging/type requested by the provider organization.

 Amount Outstanding

This column displays the difference between original amount requested and the total of all doses drawn down and rejected for this trade name/packaging/type of vaccine.

Changes Doses Requested To

This column  the new quantity can be requested with the originally submitted prebooked request.                                                                                                                         

Clicking the  Submit Updates  button will submit the updated vaccine pre-book request.

Clicking the  Delete Vaccine Request button will display a pop-up message for user to select  Ok  for successful deletion or Cancel  to return to the Modify Vaccine Request screen.

Clicking the Cancel button will display the Vaccine Request Status screen .
