User First Name

Required.  Free-text field with no default value.  Minimum length is 1; maximum length is 20.  Alphanumeric characters are only allowed.  Users First Name.

User Last Name

Required.  Free-text field with no default value.  Minimum length is 1; maximum length is 30.  Alphanumeric characters are only allowed. Users Last Name.

User Middle Initial

Length is 1; any character allowed. Users Middle Initial

Street Address

Maximum length is 55; any character allowed. Users Street Address.

Other Address

Maximum length is 55; any character allowed. Users Other Address.

P.O. Box

Maximum length is 55; any character allowed. Users PO Box.

City

Maximum length is 52; any character allowed. Users City.

State

This is a drop-down box containing all values as in ALERT IIS. Users State.

ZIP

From 1 to 5 digits (required), then from 1 to 4 digits (not required). Users Zip Code

Email

Minimum length is 1; maximum length is 80. Users Email Address.

Phone Number

Area Code: 3 digits.  Prefix: 3 digits.  Suffix: 4 digits. Users Phone Number.

Extension

Up to 7 digits. Users Extension.

Clicking on Save will validate the information entered on the screen.  A message will appear at the top indicating the status, such as if the User was saved or if there are missing required fields.

Clicking on Cancel will discard any changes made on the screen.  A confirmation message box will appear asking if you want to continue.  Pressing OK on the message box will take you back to the previous page; Cancel will keep you on the screen without discarding changes.
