Doses Administered Screen

 Purpose

The request doses administered screen exists so that an administrator can generate and print a report containing information on the doses administered that have taken place within an organization or within individual sites.

 Functionality

The Doses Administered report will produce doses administered data only for those sites that have entered non-historical immunization data into the system. In the event a site is selected that has no vaccine usage data, a message informing the user will be displayed on the report.

  1. To produce a Doses Administered report, select the desired funding source option from the Funding Source pick list. Listed below are the descriptions for each option:

Description

All Lot Numbers

This report format produces a single table containing aggregate vaccine usage data for all lot numbers within the provider organization.

All State-Supplied

This report format produces a single table containing aggregate vaccine usage data for all state-supplied lot numbers within the provider organization.

All Privately Purchased

This report format produces a single table containing aggregate vaccine usage data for all privately purchased lot numbers within the provider organization.

  1. Enter the Report Date Range by typing start/end dates into the From and To text boxes in the format MM/DD/CCYY. Or, click on the calendar icon to select the date.

  2. Click on the Generate Report button.

  3. The system will process the report and then display the report in Adobe Acrobat. *Note*

If the computer does not have Adobe Acrobat Reader version 6.0 or greater installed on it, then please see the "**REPORT VIEWING REQUIREMENTS**" notification on the New Patient Form screen, or contact your administrator.
