Reminder Request Output Options Screen

 Purpose

The Reminder Request Output Options screen is used to see the step by step details of the reminder request, such as how many patients exist in the organization and how many will be included in the report and is also for selecting the final format of the report to be printed resulting from the submission of a report query from the request reminder screen.

This section displays all the options of displaying as a report the Reminder Requests for the specified criteria described below in a table.

Output

This column displays the types of reports that can be produced based on the returned results of the Reminder Recall Query.  These reports are described in detail in the table below.  Clicking the Hyperlink in the Output column will generate the report that was clicked.

Description

This column provides a brief description of the output option.

Additional Input

This column displays the following options for adding additional information to the report and defining a report name with the text boxes described below.

  • Report Name: Enter the Report Name to describe the report that is going to be ran.

  • Free text: Enter in text that will display on the report that is going to be ran.

  • Phone #: Enter in the phone number that will display on the report that is going to be ran.

Report Output Options

Description

Reminder Letter

This report output produces a standard form letter with sufficient room at the top of each page for your provider organization's letterhead. The body of the letter includes the patient's immunization history, recommended immunizations, and can also include the free text and/or phone number.

Reminder Card

This report output produces a standard form card . The body of the card includes the patient's recommended immunizations, and can also include the free text and/or phone number.

Mailing Labels

This report produces mailing labels and will print in the same order as either of the above two reports. The report has been formatted to print on Avery Mailing Labels #5160, which contain 30 labels per page.

  • The patient's name is included in small font under the responsible person on the Mailing Labels.

  • Default sort will be on patient's last name.

Patient Query Listing

This report is produced for the administrator's records. This report will list every patient that was returned in the report query process. Along with each patient, the report will also list the phone number and full address of every responsible person associated with each patient. Any incomplete or blank lines found in this report represent insufficient or missing phone numbers and/or address information for a responsible person.

Extract Client Data

This report will produce a XML format of client, responsible person, and immunization information for all patients that are returned in the report query process.

The custom letter reports were created through manage custom letters.

 Last Notice Date Options Section

This section displays options of updating patient reminder recall information for patients associated with the reminder recall.

Command Buttons

Limitations

Clicking the Preview Patients button will display all the patients that meet the criteria for the Reminder Report on the Preview Patients screen.

None.

Clicking the Increment Patients button will increment the count of reminder recall notices for all the patients that met the criteria for the Reminder Recall report.

Only displays for Statewide Recall Option.

Clicking the Increment Eligible button sets the last notice date for all the patients that met the criteria for the Reminder Recall report.

None.

Clicking the Increment Immunized button sets the last notice date for all patients that met the criteria for the Reminder Recall report that have been immunized by the organization running the report.

None.

Clicking the Cancel button will return the user to the previous screen.
