Displays the Patient's Last Name, First Name and Middle Initial as a hyperlink. Clicking hyperlink will display the Edit patient screen for that patient. |

Displays the Birth Date for the patient in the corresponding row. |

Displays the Address for the patient in the corresponding row. |

City |
Displays the City for the patient in the corresponding row. |
