The State Prebook Report Request screen allows State users to create reports and monitor seasonal Flu requests.

Purpose |
The State Prebook Report Request screen allows the user to define the report criteria by Date Range, Date Type, and Organization.

Functionality |
Generating Prebook Report - To produce a State Prebook report, select the desired options. Listed below are the descriptions for each option.

Options |
Description |
| From and To Date | Enter the report from and to date range in MM/DD/YYYY fromat. |
| Date Type | Select the Date Type criteria: Vaccine Request Date, Allocation Date Draw-Down or Shipped Date. |
| Select Organizations |
The All VFC Organizations option is selected by default and will return all Organizations with the VFC status "Active" or "Suspended". |
| Organization Selection | Selecting Individual Organization allows the user to select organizations to be included in the report. |
| Select Vaccine Type | By default, Vaccine Type will contain all Vaccine Type values that exist on the Edit NDC Information screen. Values are VFC, Flu Pool and Pandemic. |
| Select Report Type | User can select from 3 Report Type options: Summary, Detailed or Both. |
| Generate Report button | To execute report select the Generate report button. |
