Manage Clinicians Screen

Purpose

The manage clinicians screen is used to enter new and update existing clinicians within the provider organization. Any clinician entered on this screen will be available for selection of Ordering Authority or Administered By on the Immunizations screen and/or the Record Immunizations screens. Only users with the Administrator role can access the manage clinicians screen.

Information Provided

Site List section- Site List section allows the user to select a site to display clinicians, to display all clinicians for the Organization, to find a clinician, and to add a clinician.

- Site List pick list displays all sites associated with the Organization.

1. - Sites that are left justified in the pick list have their own inventory.

1. - Sites that are center justified in the pick list do not have their own inventory.

- Functionality

1. - Selecting an option from the Site List pick list displays the clinicians associated with that site in the Clinician Table section displayed below the Site List section.

1. - Clicking the Add Clinician button displays the Edit Clinician Information screen as blank to enter a new Clinician.

1. - Clicking the Find Clinician button displays the Clinician Search screen.

1. - Clicking the Clinician List button displays the Clinician Listing screen.

Clinician Table section- Clinician Table section displays Clinicians associated with the Site that is selected in a table described below and allows the ability to edit clinician information.

Column	Description
Clinician Name	Displays the Clinician Name as a hyperlink. Clicking on the hyperlink displays the Edit Clinician Information screen populated with the clinician information.
Role	Displays the Role associated with the Clinician.
Signature	Displays if Signature of the Clinician is on file.

Add/Edit Clinician Screen

Edit Clinician Information section- This section contains primary clinician information described below.
- Selecting a Role for the clinician is required.
- Select a Role for the clinician by clicking the radio button for the Role.
- - Roles that can be associated with the clinicians are
  - - Clinician- A clinician assigned the Clinician role is an individual within a provider organization who can physically immunize a patient. In the event a clinician has an Clinician role, his or her name will be available for selection in the 'Administered By' pick list on the Pre-select Immunizations screen when entering new immunizations.
    - Ordering Authority/Clinician- When the Ordering Authority/Clinician option is selected for a clinician, this person will have the Clinician and Ordering Authority roles assigned to them. In addition, his/ her name will be made available for selection in both the 'Administered By' and 'Ordering Authority' pick lists on the Pre-select Immunizations screen when entering new immunizations.
    - Ordering Authority- A clinician assigned the Ordering Authority role is an individual within a provider organization who is licensed by the state of Oregon to authorize the giving of immunizations to a patient. In the event a clinician is given an Ordering Authority role, his or her name will be made available for selection in the 'Ordering Authority' pick list on the Pre-select Immunizations screen when entering new immunizations.
- Enter in the Clinician Name information below the Role information in the fields described in the table below.
- Selecting a Role for the clinician is required.
- Select a Role for the clinician by clicking the radio button for the Role.

In addition, his/ her name will be made available for selection in both the 'Administered By' and 'Ordering Authority' pick lists on the Pre-select Immunizations screen when entering new immunizations.
    - Ordering Authority- A clinician assigned the Ordering Authority role is an individual within a provider organization who is licensed by the state of Oregon to authorize the giving of immunizations to a patient. In the event a clinician is given an Ordering Authority role, his or her name will be made available for selection in the 'Ordering Authority' pick list on the Pre-select Immunizations screen when entering new immunizations.

Fields	Description
Prefix	Prefix of the Clinician.
Last Name	Required field. Last Name of the Clinician.
First Name	First Name of the Clinician.
Middle Name	Middle Name of the Clinician.
Suffix Name	Suffix of the Clinician.
Site Selection	Displays if Signature of the Clinician is on file.

- Selecting a Site for the clinician to be associated with is required.

1. - Select a Site for the Clinician to be associated with by double clicking a site in the Complete Site Listing pick list or by clicking the Site to highlight the site and clicking the Add button.

1. - Select all Sites for the Clinician to be associated with the Clinician by clicking the Add All button.

1. - To Remove a Site for the Clinician to be not associated with anymore double click the site in the Selected Sites pick list or by clicking the Site to highlight the site and clicking the Remove button.

1. - To Remove all Sites for the Clinician to be not associated with anymore click the Remove All button.

Address Information section- The Address Information section contains Clinician address and contact information described in the table below.

Fields	Description
Street 1	Address of the Clinician.
Street 2	Address of the Clinician usually contains suite # or more Address information if needed.
PO Box	PO Box of the Clinician.
City	City of the Clinician for address purposes.
State	State of the Clinician for address purposes.
Zip	Zip code of the Clinician for address purposes.
Email	Email address of the Clinician.
Area Code	Area Code of the Clinician for the phone number being provided.
Phone Number	Phone Number of the Clinician.
Ext	Extension of the Clinician at the phone number provided.

Signature Information section- Displays with Signature not on file, because capturing signatures for the clinicians has not been implemented for the ALERT IIS.

Command Button Functionality- Functionality of the command buttons on the Edit/Add Clinician page are described in the table below.

Command Button	Description
	Clicking the Add button will move the selected site in the Complete Site Listing pick list to the Selected Sites pick list.
	Clicking the Add All button will move all sites in the Complete Site Listing pick list to the Selected Sites pick list.
	Clicking the Remove button will move the selected site in the Selected Sites pick list to the Complete Site Listing pick list.
	Clicking the Remove All button will move all sites in the Selected Sites pick list to the Complete Site Listing pick list.
	Clicking the Save button will save the validate all required fields have been entered and will then save the clinician if all required fields are entered, otherwise will display a message to correct the required fields.
	Clicking the Delete button will inactivate the clinician.
	Clicking the Cancel button will discard all changes and or additions made to the clinician record and will return the user to the previous screen.

Find Clinician Screen

Clinician Search section- This section contains search fields described below as well as command buttons described below as well.
- Search Fields

Fields	Description
Last Name	If entered searches clinicians with the start of the last name criteria that was entered.
First Name	If entered searches clinicians with the start of the first name criteria that was entered..

- Command button functionality

Command Button	Description
	Clicking the Find button will search for clinicians associated with the organization with the criteria that was entered in the Last Name and First Name fields.
	Clicking the merge command button with two or more clinicians selected in the Search Results section will display the Clinician Merge screen.
	Clicking the Cancel button will display the previous page.

Clinician Listing Screen

Clinician Listing Table- This table displays the Clinician specific information as it pertains to sites and roles as described below.

Column	Description
Clinician Name	Displays Clinician Names associated with the Provider Organization.
Associated Sites	Displays all sites that are associated with the Clinician.
Role	Displays the Role of the Clinician.

Command Button Functionality- Describes the functionality of clicking the command buttons below.

Command button	Description
	Clicking the Close button displays the previous screen.

Clinician Merge Screen

Clinician Merge Section- This section allows the user to merge clinicians together for an organization for consolidation reasons the command buttons in this section are described below.

Command buttons	Description
	Merges all clinicians in the Merge Clinician table below into the single clinician that has been selected in the table.
	Returns the user to the previous screen.

Merge Clinician table- Displays clinician information to be able to select a clinician from the list to be the record that is kept when merging the records.

Columns	Description
Select	Displays radio buttons for each clinician to select the clinician to keep when merging the clinician records..
Clinician Name	Displays the Clinicians Last Name, Clinicians First Name as a hyperlink. If the Clinicians Name hyperlink is clicked the Edit Clinician Information page will be displayed.
Role	Displays the Role of the Clinician.
Active	Displays whether a Clinician is active for the Organization.
