Vaccine Request Status

Event

Clicking the drop down list, all currently open vaccine events will display.

 Select Box

Clicking the select check boxes will be used with the Draw Down and Edit Vaccine Request buttons to navigate to other screens. and to be able to select the vaccine lot to be able to Modify the Quantity on Hand for the vaccine lot if needed

Trade Name

This column displays the Trade name of the vaccine request.

Packaging

This column displays the  Packaging information for the vaccine.

 Manufacturer

This column displays the manufacturers information of the vaccine request .

  Type

This column displays the type of  vaccine e.g. VFC, Flu Pool and Pandemic.

    Initiating User

This column displays the user first and last  name  who submitted the original or most recently updated version of the vaccine request.

     Request Date

This column displays the original date the vaccine request was submitted.

 Request Status

This column displays the Status of the vaccine request. E.g. Substitution Prebooked or Late.

Place in Queue

This column displays the place in queue of the specified vaccine request among other providers in the State, E.g.  4th of 100 or 30th of 300 in line to receive doses.  

Total Amount  Requested

This column displays the amount of doses requested for this particular place in the queue.

 Total Amount Allocated

This column displays the total amount of doses allocated by the state for this particular vaccine request. Total Amount  Allocated is a hyper link.

   Total Drawn Down

This column displays the total number of doses drawn down.

  Amount Available Now

This column displays the difference between Amount Allocated and Amount Drawn Down.

Clicking the  Draw Down button will display the Provider Draw Down screen, it will take doses from current vaccine allocation.

Clicking the Edit Vaccine Request  button will display a pop-up window to change the previously requested amount.

Clicking the Cancel button will display the previous screen the user was on.
